Health Alliance Update - Please read carefully.

We get calls everyday asking whether we are part of the Health Alliance HMO.  In short, no we are not part of the HMO but we are working on it.  We do accept Health Alliance PPO and CCP plans. 

We do not mind answering your questions about this subject but it would be more productive if you call your HR representative or the Health Alliance customer care to request Dr. Svientek and Thornton's addition to the HMO.

From the Parking Garage..

Follow the skywalk to the end and make a left. Walk past the elevators and make a left at the hospital information desk. Follow the skywalk to the Physician's Office Building (POB). Turn right at the end of the POB skywalk. Dr. Svientek's office is on the right, Suite 270.

Call the office if you get lost.
